3. To apply for membership card , members are required to fill in an application form ( AMS Rev1-2016) and submit two copies of photographs ( identity card size , in duty uniform / SJAM T-shirt ), a photocopy of Identity Card and a photocopy of Basic First Aid Cert (latest).
